P-OGC46 Physiotherapy Regimens in Esophagectomy and Gastrectomy: a Systematic Review and Meta-analysis

Abstract Background Esophageal and gastric cancer surgery are associated with considerable morbidity, specifically postoperative pulmonary complications (PPC), potentially accentuated by underlying challenges malnutrition cachexia affecting respiratory muscle mass. Physiotherapy regimens aim to increase the strength may prevent morbidity. This meta-analysis aims assess impact of physiotherapy following esophagectomy or gastrectomy. Methods An electronic database search was performed in MEDLINE, EMBASE, CENTRAL, CINAHL Pedro databases. A on functional capacity, incidence PPC in-hospital mortality rate, Length Hospital Stay (LOS) Health-Related Quality Life (HRQoL). Results Seven RCTs 7 cohort studies assessing prehabilitation totalling 960 patients, 5 peri- 703 total were included. Prehabilitation resulted a lower pneumonia morbidity (Clavien-Dindo score >II). No difference observed exercise prehabilitation. Meanwhile, rehabilitation pneumonia, shorter LOS better HRQoL scores for dyspnea physical functioning, while no differences found QoL summary score, global health status, fatigue pain scores. Conclusions Our shows that implementing an intervention be beneficial both preoperative period. Further investigation is needed understand mechanism through which interventions improve clinical outcomes subgroup patients will gain most benefit.
